Job summary

Inova Health Center seeks a dedicated Registered Nurse (RN) Patient Care Navigator - Structural Heart in Fairfax, VA. This full-time role operates Monday–Friday with no holidays or weekends, relocation assistance available.

The RN will coordinate care, facilitate referrals, and support patients through the continuum of care. The position requires a Bachelor’s in Nursing, Virginia RN licensure, BLS, and 3 years of nursing experience, with preferred cardiac exposure.

Qualifications

• Bachelor's degree in Nursing required.
• Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support from the American Heart Association.
• 3 years of nursing experience required.

Responsibilities

• Coordinates a patient's plan of care including appointment scheduling, attendance confirmation, transportation, referrals, and patient questions.
• Acts as liaison and point of contact to help patients access services and achieve optimal healthcare outcomes.
• Refer cases to appropriate resources; anticipate needs and advocate for patient rights and service utilization.
• Conducts comprehensive health and psychosocial assessments and develops a case management plan with patient and support system.
• Educates patient, family, or caregiver on treatments, options, benefits, and self-care to support informed decisions across the continuum.
• Collaborates with multidisciplinary teams to address questions about care and treatment options.
• Develops and maintains systems to document, track, and monitor patient data.
• Performs other duties as assigned.
