Job summary

Yale New Haven Health is seeking a Nurse Coordinator to support the Structural Heart and Cardiac Surgery programs. You will coordinate the care plan for patients, communicate with the care team, and collaborate with physicians and advanced practitioners to enhance patient outcomes.

The role requires a Bachelor's in Nursing and CT RN licensure, with 2+ years of relevant clinical experience and strong data analysis and MS Office skills. Travel across sites may be required.

Qualifications

• Bachelor's Degree in Nursing is required; Master's degree strongly preferred.
• Minimum 2 years of relevant clinical experience in structural heart or cardiac surgery is desirable; 3–5 years ideal.
• Experience in data analysis, reporting and effective presentation; proficient with Microsoft Office including Excel/PowerPoint.

Responsibilities

• Develops overall care plan and communicates it to the patient care team.
• Provides direct and indirect care to patients and families; liaises with hospital staff and community agencies.
• Applies research findings to nursing practice and monitors outcomes for quality improvement.
• Attends seminars and maintains department standards; performs rounds and collaborates with mid-level practitioners.
• Supervises staff in Cardiac Surgery and Structural Heart Programs; evaluates staff performance.
