Job summary

Ideal candidates hold a Bachelor’s in Nursing, are licensed RNs with multi-state eligibility, and have 3+ years of nursing experience including ambulatory care or case

Qualifications

• Bachelor's degree in Nursing required.
• RN license with multi-state eligibility required.
• 3+ years of nursing experience; ambulatory care/cardiac navigation experience preferred.
• BLS certification required; cardiac patient navigation preferred.

Responsibilities

• Assess needs for new patients referred to a cardiology specialist before first visit.
• Document patient assessments and interventions in the EMR with accuracy.
• Refer patients to program services based on navigation needs.
• Educate and support patients throughout intake and treatment planning.
• Coordinate scheduling with multidisciplinary providers and locations.
• Assist in arranging diagnostic tests to expedite treatment start.
• Develop relationships with referring physicians and ensure follow-up communication.
• Contribute to program metrics reporting and quality initiatives.
