HCA Florida Oak Hill Hospital is seeking a Structural Heart Program Coordinator RN to work with physicians and nurse practitioners to coordinate care pre- and post-procedure, inpatient and outpatient settings.

The role emphasizes patient experience and exceptional service, with duties including coordinating referrals, testing, and ongoing follow-up while supporting staff education and program outreach.

Qualifikationen

• Current licensure in Florida as a Registered Nurse; BLS required from AHA or American Red Cross.
• BSN required; MSN preferred.
• Background in cardiac or pulmonary and strong skills in coordination, facilitation, patient communication and physician relationships.
• Ability to use Microsoft Word, Excel, and data entry; ability to multi-task.

Aufgaben

• Receive and review referrals for appropriateness and coordinate with an attending as needed.
• Obtain outside imaging and medical records.
• Coordinate initial work up and diagnostic testing for patients and families; assist with lodging for out-of-town families.
• Perform assessments and initial work up for patients.
• Coordinate testing and review with cardiologist and CTS at valve clinic visits.
• Develop treatment or management plans with the team and assist in clinics.
• Educate patients and families.
