Job summary

Baptist Health is seeking a Structural Heart Nurse Coordinator in Lexington for full time, first shift. The role leads patient triage, care coordination across the valve disease continuum, and collaboration with cardiology and surgical teams to optimize outcomes.

The position requires nursing excellence, leadership, and a solid cardiology/ICU background to drive program development, patient experience, and regulatory compliance.

Qualifications

• Bachelor’s degree in nursing from an accredited school.
• Minimum of three years of experience.
• Experience in Cardiology and/or Intensive Care required.
• Previous leadership and experience with projects.
• Basic Life Support certification via the American Heart Association within 90 days of hire.

Responsibilities

• Acts as the point of contact for patients and families, starting with the initial contact, telephone triage and screening.
• Guides patients through the continuum of care from admission to 30-day and 1-year follow-up.
• Ensures continuity of care through optimized assessment, triage, communication with physicians, and daily management.
• Collaborates with interventional cardiologists and cardiothoracic teams to coordinate care for valvular disease and related procedures.
• Supports program development, marketing, screening initiatives, and performance improvement.
