Job summary

Yale NewHaven Health is seeking a Nurse Coordinator to coordinate care for structural heart, cardiac and thoracic surgery patients. You will work with APPs and physicians to optimize care, quality metrics, and outcomes across the Heart and Vascular Center.

The role emphasizes patient-centered care, clinical leadership, data analysis and performance improvement initiatives within a busy hospital setting.

Qualifications

Bachelor's Degree in Nursing is required.
Master's Degree strongly preferred.
Minimum of 2 years of relevant clinical experience; 3–5 years ideal.
Experience in data analysis, reporting and effective presentation; proficient in Microsoft Office including Excel and PowerPoint.

Responsibilities

Develops the overall plan of care/guidelines and communicates plan to the patient care team.
Provides direct and indirect care to patients and families.
Acts as liaison between patients, families, hospital staff and community agencies to promote communication and discharge planning.
Utilizes relevant research findings and quality outcomes to support and advance nursing practice and improve patient outcomes.
Attends educational seminars to maintain and meet expectations set forth by Hospital and departmental standards.
Performs daily in-hospital rounds and weekly meetings with mid-level practitioners to follow inpatients through their hospital stay.
Provides direct supervision to staff in the Cardiac Surgery and/or Structural Heart Programs.
