Job summary

Northwest Medical Center in Tucson seeks a Patient Navigator - RN for the Structural Heart program working in the Cath Lab. This full-time role guides patients and families through the continuum of care and coordinates logistics to reduce barriers to treatment.

You will assess needs, educate patients on procedures and post-care, and collaborate with a multidisciplinary team to ensure timely treatment and smooth transitions.

Qualifications

• 3-5 years of clinical nursing experience.
• 1-3 years of patient navigation, care coordination, or case management preferred.
• RN - State Licensure required.
• BCLS/Basic Life Support certification required.

Responsibilities

• Serve as a primary contact for patients, families, and caregivers from diagnosis through treatment.
• Assess patients' medical, emotional, and socioeconomic needs and address barriers to care.
• Provide patient education on treatment, side effects, and care plans.
• Facilitate communication between patients, caregivers, and healthcare providers.
• Coordinate referrals to social workers, financial counselors, dietitians, and community resources.
• Assist patients with insurance-related matters and obtaining financial assistance.
