Job summary

Baptist Health Lexington is seeking a Structural Heart Nurse Navigator to guide patients from first contact through post-procedure follow-up. You will coordinate care across inpatient and outpatient settings, ensuring seamless communication with physicians and care teams.

Ideal candidates hold a KY/Compact RN license, have 3+ years in cardiology or ICU, and bring leadership or project experience to help grow the structural heart program and screening initiatives.

Qualifications

• Current Kentucky or Compact State unrestricted license as a Registered Nurse.
• Basic Life Support via the American Heart Association within 90 days of hire.
• Three Years of Relevant experience
• Experience in Cardiology or Intensive Care preferred
• Precious leadership experience or experience with projects preferred

Responsibilities

• Serve as the primary contact for patients and families from initial contact through follow-up.
• Coordinate care across inpatient, 30-day and 1-year follow-ups to ensure continuity of care.
• Collaborate with interventional cardiologists and cardiothoracic surgeons to manage TCAV/TVC patient pathways and program development
