Job summary

Yale New Haven Health is seeking a Nursing Coordinator to coordinate care for structural heart, cardiac and thoracic surgery patients, partnering with physicians and APPs to drive value and outcomes.

The role focuses on patient care, operational metrics, documentation of care plans, and leading staff within the Cardiac Surgery/Structural Heart programs.

Requires BSN, Connecticut RN licensure, 2–5 years clinical experience, data analysis and MS Office proficiency; Master’s degree preferred.

Qualifications

• Bachelor's Degree in Nursing required; Master's Degree strongly preferred.
• Minimum of 2 years of relevant clinical experience; 3–5 years ideal.
• Proficient in data analysis, reporting and effective presentation; MS Office skills.

Responsibilities

• Develops the overall plan of care and communicates the plan to the care team.
• Acts as liaison between patients, families, staff and community agencies.
• Utilizes research findings to support and advance nursing practice and outcomes.
• Performs daily rounds and weekly meetings with mid-level practitioners.
• Provides supervision to staff in Cardiac Surgery/Structural Heart Programs.
