Job summary

Grady Health System is seeking a Nurse Stroke Navigator to improve patient preparedness for treatment through education and psychosocial support, and to coordinate care among patients, physicians, and referrals. You will contribute to program development, expansion, and achievement of goals, ensuring clear communication of follow-up needs to patients.

BSN in Nursing and Georgia RN licensure are required, along with BLS.

Qualifications

• BSN in Nursing required.
• Registered Nurse licensed in Georgia required.
• BLS required.
• 3+ years job-related experience, Neuroscience preferred.
• SCRN preferred.

Responsibilities

• Coordinate patient education and psychosocial support for treatment readiness.
• Facilitate interaction between patients, physicians, and referrals as appropriate.
• Assist in development of educational materials and program monitoring.
