Job summary

Grady Health System is seeking a Nurse Stroke Navigator to improve patient preparedness for treatment through education and psychosocial support, and to coordinate care among patients, physicians, and referrals. You will contribute to program development, expansion, and achievement of goals, ensuring clear communication of follow-up needs to patients.

BSN in Nursing and Georgia RN licensure are required, along with BLS.

Qualifications

• BSN in Nursing required.
• Registered Nurse licensed in Georgia required.
• BLS required.
• 3+ years job-related experience, Neuroscience preferred.
• SCRN preferred.

Responsibilities

• Coordinate patient education and psychosocial support for treatment readiness.
• Facilitate interaction between patients, physicians, and referrals as appropriate.

Job Summary

The Nurse Stroke Navigator improves patient preparedness for treatment through education and psychosocial support while also facilitating interaction between patients, their physicians, and referrals as appropriate. The Nurse Stroke Navigator assists in the development of appropriate educational materials and program monitoring to promote coordination of care and patient satisfaction. The Nurse Stroke Navigator assists the clinical and leadership team with program development, expansion and successful achievement of programmatic goals. The Nurse Stroke Navigator will communicate effectively with patient ensuring seamless plan of care and understanding of follow up needs.

BSN in Nursing Required

Registered Nurse in the State of Georgia required

BLS

3 or more years job related experience, including experience in Neuroscience preferred

SCRN preferred
