Job summary

Appalachian Regional Healthcare (ARH) is seeking a stroke coordinator to lead and coordinate stroke services across the community. The role acts as a liaison among physicians, nursing staff, allied health professionals, patients, and families to ensure adherence to clinical guidelines and seamless care.

Qualifications

  • Assists with the coordination of multidisciplinary care for stroke patients in collaboration with LIPs and nursing.
  • Provide clinical resources and consultation to staff regarding stroke patients with clinical complex needs.
  • Works with department managers and directors to implement formal and informal nurse and hospital education and training regarding stroke care.
  • Assists with development and coordination of community education for healthcare professionals and lay community.
  • Completes chart reviews to ensure compliance with stroke accreditation guidelines.
  • Develops and implements initiatives to improve care, in coordination with stroke program director.
  • Works with Stroke Medical Director in promotion and marketing of the stroke program.
  • Attends and participates in local quality management system meetings.
  • Attends and participates in monthly ARH system stroke coordinator meetings.
  • Attends and participates in quarterly Norton/UK Stroke Program Coordinator meetings.
  • Represents and promotes stroke services to hospital staff, volunteers, patients and visitors through a positive attitude and effective communication skills.
  • Exhibits professional conduct and demeanor during all interactions.
  • Acts as a liaison between LIPs, nursing departments, and ancillary staff on behalf of patients and their families.
  • Ensures that stroke care is consistent with current evidence-based best practice.
  • Leads and/or assists with coordination of performance improvement program initiatives, including outcome evaluations and action planning as necessary.
  • Communicates with leadership regarding any issues regarding service, personnel or patients.
  • Leads local stroke committee meetings (quarterly at minimum) and provides program performance reports at facility leadership meetings.
  • Ensures submission of quality metrics to Norton/UK Stroke Care Network on or prior to the quarterly due date.
  • Maintains current certification in the NIHSS and other required regulatory components to the position.
  • Will complete a minimum of eight hours each year in stroke specific continuing education.

Responsibilities

  • Associate Degree in Nursing Required
  • Bachelor’s degree in nursing Preferred
  • 1-3 years work in ED/ICU/acute care setting required
  • 1-2 years’ experience in process improvement or leadership preferred
