Job summary

Central City Concern's Warm Welcome Program conducts direct street outreach to people experiencing homelessness. As part of the multidisciplinary team, the Community Health Outreach Worker engages with individuals to facilitate connections to healthcare, shelter resources, and housing options.

Qualifications

• High school diploma or GED required.
• 1 year of experience including work in human services, community health, social services, volunteer work, and internships.
• Must possess a current driver's license, pass a DMV background check and be designated an acceptable driver.
• Must pass CPR certification prior to start.
• Must understand causes of homelessness and related health and social service systems.
• Must adhere to Drug-Free Workplace policy and not use controlled substances in the workplace.
• Must pass pre-employment drug screen, TB Test, and DHS background check.

Responsibilities

• Work with Warm Welcome Street Outreach team to engage people experiencing unsheltered homelessness in East Portland.
• Provide direct outreach to link participants with healthcare and shelter services.
• Maintain a prioritized list of participants and updated charts.
• Assist participants in engaging with internal and external system resources.
• Provide transportation for program-related appointments when needed.
• Attend mandatory trainings and participate in quality improvement activities.
• Ensure privacy compliance including HIPAA and 42 CFR Part 2.
