Job summary

Shelter House, Inc. is hiring an Outreach Case Manager for the Kennedy Emergency Shelter. The role focuses on street outreach, engagement, and housing-first services for unsheltered individuals in Fairfax County's Region 1.

The position requires trauma-informed and culturally responsive engagement, with opportunities to collaborate with partners and education in providing access to housing and supportive services.

Qualifications

• Bachelor's degree in human services or commensurate experience.
• 2+ years of case management or direct services experience.
• Strong written and oral communication skills.
• Ability to prioritize competing priorities and make sound judgments.
• Ability to deliver creative, resourceful solutions to unique challenges.

Responsibilities

• Actively seek out and engage unsheltered individuals across Fairfax County's Region 1 locations.
• Maintain a consistent schedule of street outreach stops at drop-in centers, campsites, hypothermia shelters, and other locations.
• Provide housing-first case management to move unsheltered individuals into permanent housing.
• Collaborate with the multidisciplinary Homeless Healthcare Program (HHP) and community partners to connect clients to services.
• Document all services and referrals, and maintain HMIS in compliance with standards.
