Job summary

Shelter House, Inc in Virginia is seeking a Kennedy Shelter Case Manager to provide dedicated case management for single clients at Kennedy emergency shelter. You will support housing stability through trauma-informed, housing-first approaches within a collaborative team environment.

Responsibilities include assessments, service planning, coordinating housing/financial services, and maintaining HMIS records while working flexible hours, including some evenings or weekends as needed.

Qualifications

• A bachelor's degree in human services/ related field or commensurate experience.
• 2+ years of case management/ direct services/ related experience.
• Strong written and oral communication skills.
• Ability to prioritize competing priorities and make sound judgments.
• Ability to complete tasks while navigating frequent interruptions.
• Ability to deliver creative, resourceful solutions to unique challenges.
• Strong organizational and time management skills.
• Valid Driver's License and reliable transportation.
• Ability to drive a 15-passenger van.
• Willingness to work evenings and weekends when needed.

Responsibilities

• Use a strengths-based approach to assess client strengths, health, social and education needs.
• Determine support needed to maintain stability with client involvement.
• Develop and monitor Individualized Service Plans.
• Coordinate services (employment, housing, finance, mental health, SSI) for clients.
• Provide crisis support and coping skills training.
• Create and manage budgets and rent/utilities payments.
• Maintain complete client records per agency standards.
• Document client contacts in HMIS and complete reassessments.
• Participate in staff meetings and trainings as directed.
• Ensure compliance with agency policies.
• Collaborate with multidisciplinary team across Shelter House Inc. and partners.
• Disseminate relevant information to appropriate staff promptly.
• Utilize Microsoft Office as part of daily duties.
• Safely transport clients using Shelter House vehicles when needed.
