Job summary

Healthcare in Action, Inc. is hiring a Peer Navigator to support members experiencing homelessness in San Fernando Valley, Duarte and surrounding areas. This patient-facing role focuses on education, peer support, housing & resource navigation, and coordination with social services.

The position offers a base hourly rate of $21.78–$33.00, with a travel requirement of up to 50%. It includes an annual bonus program, generous PTO, health benefits, a 401(k) with company match, tuition reimbursement,

Qualifications

• High school diploma or GED required with 3+ years of relevant work experience
• Associate's degree plus 2+ years of experience OR Bachelor’s degree plus 1+ year experience preferred
• Experience with homelessness outreach or similar communities is a plus

Responsibilities

• Build rapport with members to encourage positive behavioral changes
• Coach members to implement strategies for risk reduction
• Connect members to local programs and resources for care
• Coordinate or accompany members to appointments outside the street medicine unit
• Document encounters and update care plans in the system
