Job summary

SYNERGY HomeCare is seeking a Community Care Liaison to drive community engagement, build referral partnerships, and sign clients through in-home assessments and outreach across three geographic areas.

You will coordinate live events, cultivate relationships with referral sources, maintain contacts, and report outcomes to support growth and revenue goals. This is a field-based, relationship-driven role with performance bonuses.

Qualifications

• High School Diploma or GED required.
• At least four years of outside sales, event planning, or similar responsibilities.
• Proven success in building relationships that contribute to revenue goals.
• Active listener and effective communicator with excellent interpersonal skills.
• Self-motivated with ability to thrive with minimal supervision.

Responsibilities

• Deliver best-in-class, mission-driven live events and outreach activities such as networking groups and health fairs.
• Cultivate and create relationships with key referral sources and community partners.
• Maintain a database of contacts, track sales efforts, and report on activities and results.
• Conduct presentations, face-to-face meetings, phone calls, events, etc. with prospects and current referral sources to share information about Synergy HomeCare.
• Conduct in-home assessments and sign clients; support and grow our presence in three key geographic areas.
