Job summary

MedStar Health’s Washington Hospital Center is seeking an experienced nurse leader to plan, organize, and direct the Utilization Case Management and Discharge Planning Programs. You will supervise staff, align resources with budgets, and ensure compliance with policies and regulations to deliver safe, cost-effective patient care.

The role requires an MSN or related master’s, 3–4 years of progressive leadership experience, and an active RN license in MD or DC; CCM certification preferred.

Qualifications

• Master’s degree in Nursing or related field.
• 3–4 years of progressive leadership experience; CM/SW/UR experience preferred.
• RN license in MD or DC; CCM certification preferred.

Responsibilities

• Plans coordinates and directs the Utilization Case Management and Discharge Planning Programs.
• Monitors discharge planning services and counseling to patients and families and consultation to medical staff regarding patient management.
• Develops and implements clinical pathways, procedures and guidelines at hospital and system level.
• Directs and participates in social services including psychosocial assessment, crisis intervention, home health care, oncology and other discharge planning support groups.
• Ensures delivery of quality care by identifying patient needs and developing social services programs.
• Oversees monthly monitoring and reporting of LOS, discharges, outcomes and denials.
