Job summary

The Christ Hospital Home Health Care is seeking a dedicated Home Health Aide to provide compassionate, one-on-one care in patients' homes. You will assist with daily activities, monitor health, and communicate patient needs to the nursing team while building trust with families.

Qualifications

• STNA, CNA, or 75-hour Nurse Aide Training required.
• 1+ year of caregiving experience preferred.
• Valid driver’s license and auto insurance required.

Responsibilities

• Assist with bathing, grooming, dressing, toileting, and personal care.
• Help patients maintain a safe and comfortable home environment.
• Observe and report changes in patient condition to nurses.
• Communicate with nurses, patients, and families, and document care provided.
• Support the clinical team to improve patient outcomes and independence.
