Responsibilities

Coordinates and provides services related to the Stepping Up program.
Conducts case management services and develops needs assessments/case management plans.
Represents the needs of Stepping Up participants and ensures quality services.
Completes needs assessment and reviews needs regularly.
Develops case management service plans with client goals.
Implements case management plans and monitors progress.
Represents needs of individuals with serious mental illness and co-occurring substance use disorders.
Participates in ensuring quality of services and ongoing training.
