The SteppingUp Case Manager provides support to individuals with serious mental illness (SMI), substance use disorders (SUD), or co‑occurring conditions as they transition from incarceration back into the community. This position is part of the Stepping Up Initiative, aimed at reducing the number of individuals with behavioral health conditions in jails. The Case Manager will work with incarcerated individuals nearing the end of their sentence to provide reentry planning, support, and linkage to essential community services that promote recovery and long‑term stability.
