Job summary

AltaPointe Health in Kentucky is seeking a Case Manager to support consumers across mental and physical health, living, learning, and social environments. You will travel to consumers' settings, assess needs, set goals in ISP, and link to community resources to promote recovery and independence.

Responsibilities include ongoing monitoring, crisis de-escalation, collaboration with treatment teams, and thorough documentation per AltaPointe standards.

Qualifications

• Bachelor’s degree in behavioral health or social services; driving requirements met.
• Knowledge of adult psychiatric disorders and addiction
• Knowledge of cultural diversity
• Knowledge of behavior management and crisis intervention
• Knowledge of psychiatric rehab and addiction treatment principles
• Knowledge of community resources
• Knowledge of adult education methods for severe psychiatric disorders
• Knowledge of entitlement programs (Social Security, Medicaid, Medicare)

Responsibilities

• Assess consumer needs using SUN-R.
• Set goals and ISP plans with services.
• Link to community resources
• Monitor progress for least restrictive environment
• Advocate for resources
• Collaborate with treatment team
• Document progress notes timely per AltaPointe standards
• Manage caseload to meet productivity standards
• Provide training on basic living skills
• Assist transportation to appointments
• Make multiple weekly contacts with consumers
