Qualifications

• BLS Provider certification within 30 days of hire or transfer.

Responsibilities

• Lead the execution and monitoring of individualized patient care plans for optimal recovery.
• Document the patient care journey with meticulous detail and update the team promptly.
• Coordinate clinical disciplines to ensure cohesive patient care from admission to discharge.
• Execute physician and nursing orders promptly to maintain safety and adherence.
• Assess discharge needs early and collaborate on comprehensive transition plans.
