Qualifications

  • Proficiency in MS Office 365 preferred.
  • Strong interpersonal skills, ability to work with diverse populations and good communication skills.
  • Ability to work independently and as part of a team.

Responsibilities

  • Provide housing case management to a caseload of participants.
  • Recruit and educate landlords about the State Opiate Response Program's benefits.
  • Understand program regulations related to this position.
  • Conduct intake processes and assess client needs.
  • Create a monthly budget and a detailed action plan with all clients.
  • Assist in finding safe, affordable housing based on participant needs.
  • Help participants read and understand lease agreements.
  • Maintain accurate client files and enter data into ClientTrack.
  • Connect participants with employment services and workforce programs.
  • Assist consumers with connecting to medical and mental health supports.
  • Link participants with school districts or childcare options.
  • Conduct home and office visits with consumers and provide program reports.
  • Coordinate efforts with UNC programs and referrals.

Job description

Position Summary

The State Opiate Response (SOR) Case Manager provides supportive services to Lackawanna and Wayne County individuals who are diagnosed with an Opiate Use Disorder, to assist in finding suitable housing, and providing life skills support to participants. A strong candidate must possess comfort with excel, data entry, customer service in the form of in person and electronic correspondence.

Responsibilities
  • Provide housing case management to a case load of participants
  • Recruit and educate landlords about the State Opiate Response Program, and the benefits of participation.
  • Have a thorough understanding of program regulations related to this position.
  • Conduct intake processes, assessing the needs of individuals and families who walk in for our services.
  • Create a monthly budget and a detailed action plan with all clients.
  • Assist in finding safe and affordable housing in the community, based on the needs of the participant.
  • Help participants to read and understand lease agreements.
  • Maintain accurate client files, financial documentation, and record keeping, including entering comprehensive client information into the ClientTrack database.
  • Connect participants with employment services and/or workforce development programs.
  • Assist consumers with connecting to medical/mental health supports.
  • Link participants with various supports with the school districts or childcare options.
  • Conduct home and office visits with consumers.
  • Provide program reports.
  • Coordinate efforts with other UNC programs, including referrals.
  • Enter service and program information into ClientTrack client management system.
  • Any other duties deemed necessary by Community Services Director.

  • Experience working with individuals from diverse backgrounds.

Education/Experience/Licensure
  • High School diploma or equivalent is required.
