Qualifications

  • High School diploma or equivalent is required.
  • Bachelor degree in social work, counseling, or related field preferred.
  • Proficiency in MS Office 365 preferred.
  • Strong interpersonal skills, ability to work with diverse populations and good communication skills.
  • Ability to work independently and as part of a team.

Responsibilities

  • Provide housing case management to a caseload of participants.
  • Recruit and educate landlords about the State Opiate Response Program's benefits.
  • Understand program regulations related to this position.
  • Conduct intake processes and assess client needs.
  • Create a monthly budget and a detailed action plan with all clients.
  • Assist in finding safe, affordable housing based on participant needs.
  • Help participants read and understand lease agreements.
  • Maintain accurate client files and enter data into ClientTrack.
  • Connect participants with employment services and workforce programs.
  • Assist consumers with connecting to medical and mental health supports.
  • Link participants with school districts or childcare options.
  • Conduct home and office visits with consumers and provide program reports.
  • Coordinate efforts with UNC programs and referrals.
