Job summary

YAI - Seeing beyond disability is seeking a START Coordinator to manage a caseload of individuals and provide systemic consultation across teams. The role requires collaboration with clinical leadership, ongoing outreach, and timely documentation in START systems.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or related field.
• At least two years of experience with the I/DD population and/or behavioral health needs.
• START Coordinator Certification training within 12 months of appointment.
• Ability to travel regularly across assigned region and other states as needed.

Responsibilities

• Maintains an active caseload of 20–25 individuals referred for START Services and provides systemic consultation using best practices.
• Conducts regular outreach visits to individuals’ home, day and/or vocational settings.
• Assists with referrals for enrollment in START and completes intake assessments.
• Ensures coordination of support meetings and crisis plan development for individuals served through START.
• Develops Comprehensive Service Evaluations and crisis prevention/intervention plans for START programs.
• Enter data into START Information Reporting System and other required systems on time.
• Participates in leadership, clinical team meetings, and START Therapeutic Coaching program.
• Provides on-call support on a regularly scheduled rotational basis.

Education

Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field
