Job summary

YAI - Seeing beyond disability is seeking a START Coordinator in San Jose, CA. You will manage a caseload of 20–25 individuals, performing home and community visits, admissions intake, and cross-system collaboration to ensure effective START services.

The role requires a Master’s in a human services field and at least two years' experience with I/DD or behavioral health needs, plus START Coordinator training within 12 months. Travel and on-call duties are required.

Qualifications

• Master’s degree in Mental Health, Psychology, Counseling, Social Work or other Human Service field.
• Bachelor’s degree in human services discipline.
• Two years of experience with the I/DD population or behavioral health needs.
• START Coordinator Certification training within 12 months of appointment.

Responsibilities

• Maintains an active caseload of up to 20–25 individuals referred for START Services.
• Conducts outreach visits to individuals’ homes, day, or vocational settings (in-person or telehealth).
• Assists with enrollment referrals, intake assessments, and coordinating with clinical teams.
• Coordinates support meetings and crisis plan development for START clients.
• Develops Comprehensive Service Evaluations and outcome plans for START services.
• Participates in regular START leadership and clinical team meetings.
• Provides on-call support on a rotating basis.
• Completes documentation and data entry into START Information Reporting System.
• Engages with community partners and presents at Clinical Education Team Meetings.
• Completes trainings to maintain START Coordinator certification.
• Complies with HIPAA and regulatory requirements.
