Job summary

Turning Point Community Programs in Rancho Cordova, CA, seeks a direct service professional to support members in meeting goals toward crisis resolution and wellness in the community. Qualifications

  • Master’s degree in Social Work, Psychology, or related field and 2 years of mental health services experience.
  • Bachelor's degree in a relevant field with 4 years of relevant experience, or Master’s degree with 2 years.
  • CA driver's license and current vehicle insurance/registration.
  • Reliable transportation capable of passing vehicle safety inspection if vehicle is over five years old.

Responsibilities

  • Assess, coordinate, and facilitate the support required to help individuals and families reduce crisis events and achieve life goals.
  • Completes required paperwork and conducts assessments.
  • Develops and participates in interdisciplinary team meetings.
  • Completes tasks for fidelity to START Model and related certifications.
  • Coordinate care with internal and external partners; advocate for resources.
  • Develop individual treatment plans and provide group and individual therapy as indicated.
  • Make recommendations for improvement in the clinical program as part of the clinical team.
  • Meet clients at Turning Point sites or in the field and transport to appointments.
  • Coach clients and families to meet treatment goals.
  • Assist in Medi-Cal eligibility and continuation of benefits when appropriate.
  • Provide on-the-spot crisis counseling and prompt intervention when needed.
  • Complete paperwork promptly and maintain documentation standards.
  • Provide after-hours on-call coverage on a rotational basis.
