Job summary

Turning Point Community Programs seeks a licensed mental health professional to support Transition Aged Youth (TAY) and families in crisis resolution and wellness. Responsibilities include assessing needs, coordinating supports, and delivering individual and group therapy within START Model guidelines.

Qualifications include a Master’s degree in Social Work or related field and two years of mental health experience, plus California driver’s license and reliable transportation.

Qualifications

• Master’s degree in Social Work, Psychology, or a related field.
• Two years of varied experience as a provider of mental health services.
• California driver’s license and current vehicle insurance/registration.
• Reliable transportation capable of passing vehicle safety inspection if more than five years old.

Responsibilities

• Assess, coordinate, and facilitate supports for Transition Aged Youth (TAY) and families to decrease crisis events and support life goals.
• Complete required paperwork and assessments; maintain documentation.
• Develop and participate in interdisciplinary team meetings and START Model activities.
• Provide in-person, after-hours on-call coverage as needed.
• Transport clients to appointments and coordinate care with internal and external partners.
