Job summary

BAYADA Home Health Care is seeking a registered nurse (RN) case manager for hospice in Harrison, New Jersey. This full-time role involves visiting clients in designated territories, providing palliative care, and collaborating with a multidisciplinary team to deliver high-quality home care.

Prerequisites include an NJ RN license and at least 1 year of clinical experience; home care/hospice background is preferred. EMR fluency is a plus; travel is required.

Qualifications

• Current license as a Registered Nurse (RN) in NJ.
• Minimum of one year of recent clinical experience.
• Home care or hospice experience preferred but not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medication, wound care, treatments and comfort care.
• Monitor client conditions and report changes to supervisor.
• Follow up with and document doctors' orders.
• Perform client assessments as necessary.
• Occasional on-call after hours and/or weekends.

BAYADA Hospice is looking for a compassionate and dedicated Full Time Registered Nurses ( RN) to join our team in the Hospice office , located in North Bergen. This office services hospice clients on a per visit basis in the Hudson and Bergen.

• Compensation-$50-$52/ hour-based on experience-hospice preferred

Prior home care and/or hospice experience strongly preferred, but not required. As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Qualifications for a Hospice RN - Registered Nurse include :

• A current license as a Registered Nurse (RN) in NJ.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
