Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for Somers Point, New Jersey. This role involves visiting clients at home within Atlantic and Cape May counties and providing palliative care as part of a multidisciplinary team.

Prior home health or hospice experience is preferred, with a focus on compassionate, patient-centered care. The RN will administer medications, wound care, and treatments, document orders, and participate in ongoing training and career

Qualifications

• Current NJ RN license.
• Minimum 1 year of clinical experience (medical/surgical).
• Home health/hospice experience preferred but not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Administer medication, wound care, treatments, and comfort care as assigned.
• Monitor client conditions and report changes to the Clinical or Client Services Manager.
• Follow up on doctors' orders and ensure proper documentation.
• Perform client assessments as needed; participate in on-call after hours/weekends.

BAYADA Hospice is looking for compassionate and dedicated Full Time Registered Nurses (RN) to join our team in the Hospice office, located in Linwood, NJ. This office services hospice clients on a per visit basis in the Atlantic and Cape May counties.

Prior home care and/or hospice experience strongly preferred, but not required.

As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a Hospice RN - Registered Nurse include

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Occasional on call after hours and/or weekends

Qualifications for a Hospice RN - Registered Nurse include

• A current license as a Registered Nurse (RN) in NJ.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.
