Job summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Case Manager, Hospice for a full-time nursing position in Kearny, New Jersey, serving hospice clients in Hudson and Bergen counties. The role requires an active NJ RN license and at least one year of recent clinical experience; prior home health or hospice work is preferred.

Qualifications

• A current license as a Registered Nurse (RN) in NJ.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care and/or hospice experience strongly preferred, but not required.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.

Responsibilities

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments and procedures and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Occasional on call after hours and/or weekends

BAYADA Hospice is looking for a compassionate and dedicated Full Time Registered Nurses (RN) to join our team in the Hospice office, located in North Bergen. This office services hospice clients on a per visit basis in the Hudson and Bergen.

Compensation-$50-$52/ hour-based on experience-hospice preferred Weekly pay Full time benefits Bilingual Spanish a plus Reliable transportation required No weekends or on call required

Prior home care and/or hospice experience strongly preferred, but not required.

As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.
