Qualifications

• Pharmacy degree required; PharmD or MS preferred.
• Current IL Pharmacist license required.
• 2+ years in acute care or home infusion preferred.
• Experience in IV admixture and IV therapy.
• Collaborates with physicians, nurses, and teams.
• Strong problem solving and staff supervision skills.

Responsibilities

• Review infusion orders and patient therapies.
• Participate in clinical rounds.
• Perform sterile compounding and TPN review.
• Maintain patient records and care plans.
