Qualifications

Responsibilities

• Interprets physician’s orders for medications and ensures therapy is appropriate for patient’s diagnosis, weight, and age.
• Compounds and dispenses prescribed medications per physician’s orders in all patient care areas in accordance with departmental policies and applicable laws.
• Verifies and transcribes physician orders within the hospital information system thereby maintaining patient medication profiles.
• Verifies outpatient prescriptions and counsels patients receiving medications.
• Provides pharmaceutical care to patients through dosing guidelines, antimicrobial stewardship program, and review of lab data for dosing and levels with physician recommendations.
• Documents all clinical interventions in the patient’s profile.
• Detects, prevents, evaluates, and reports Adverse Drug Events.
• Promotes formulary use by converting non-formulary orders to formulary when possible and coordinates procurement of non-formulary drugs.
• Ensures proper technique and accurate preparation of all pharmaceutical products (oral, IV, chemotherapeutic, investigational).
• Ensures compliance with controlled substance distribution per state and federal regulation.
• Participates in medical emergencies.
• Provides drug information to health care team.
• Supervises the activities of supportive personnel.
• Assists in training of new departmental staff.
• Participates in department-specific Performance Improvement Activities.
• Performs related duties as assigned or requested.
• Able to practice in various pharmacy areas as workload requires.
• Responsible for cost-effective compounding and dispensing of medications.
• Follows Texas State Board of Pharmacy Rules and Regulations, center policies, USP 797 and NABHO standards.

Job Specific:
