Job summary

Father Bill’s & MainSpring is seeking a Stabilization Case Manager to support families moving from homelessness to housing in a full-time, community-based role. The position focuses on referrals to educational and employment opportunities and coordinating care for families over a one-year term.

The Stabilization CM helps maintain permanent housing, acts as a mediator with landlords, and ensures services align with funder requirements while promoting self-sufficiency.

Qualifications

• Bachelor's Degree or equivalent work experience.
• CPR certification is required and provided by FBMS. AED and First Aid may also be required.
• Valid driver’s license and good driving record are required.

Responsibilities

• Complete intake for all family members 18+, using a housing-first framework.
• Maintain data-based records and quarterly/ monthly reporting by funders.
• Meet with participants at least quarterly; develop and monitor individual service plans.
• Maintain accurate case files and adhere to funder requirements.
• Provide outreach and community assessments as needed.
• Collaborate with shelter, housing search, child services, and workforce teams.
• Conduct home visits and assist clients to access community resources and transport.
• Attend supervision, agency and community meetings and trainings.
