Job summary

Father Bill’s & MainSpring seeks a Stabilization Case Manager in Plymouth, MA. This full-time, hourly role provides ongoing case management to families moving from homelessness to housing, with a one-year term and referrals to educational and employment opportunities as needed.

The candidate will coordinate care, mediate housing issues with landlords, and maintain permanent housing. A Bachelor's Degree or equivalent experience is required, CPR certification is provided by FBMS, and AED/First Aid

Qualifications

• Bachelor's Degree or equivalent work experience.
• CPR certification is required and provided by FBMS.
• AED and First Aid may also be required and will be provided by FBMS.
• Valid driver’s license and good driving record required.

Responsibilities

• Complete intake process within required time frame for all family members 18+, utilizing a housing first framework and understanding the barriers to house people experiencing homelessness.
• Collect and maintain data-based statistical information, maintain accurate case records including data entry on a monthly and quarterly basis.
• Meet with participants at least once a quarter to monitor progress and implement individual service plans monthly for all family members 18+.
• Set up and maintain accurate case files and adhere to requirements determined by program funders.
• Provide outreach and assessment in the community as deemed necessary by supervisor or administrator.
• Collaborate, refer and case conference with staff including family shelter, housing search, child services, workforce development and Prevention/Diversion staff including TPP.
• Complete home visits and assist participants to access community resources providing support and transportation.
• Participate in scheduled supervision, agency and community meetings and trainings when required.
• Adhere to requirements as determined by program funders.
• Initiate crisis intervention when necessary.
• Maintain professionalism and ethics in service delivery.
