  • Monitor school attendance and progress for school-age children.
  • Guide families through first-time homeownership programs.

Salary Range: $25.00 To $27.00 Hourly

We are looking for people to partner with us to achieve our goal of disrupting the cycle of homelessness for greater Boston families and are seeking Temporary Part Time Stabilization Case Managers to join our stabilization team.

20 hours a week- $25-$27 an hour

What You Will Do:

Support families exiting Hildebrand Emergency Shelter for 24 months and in some cases up to 36 months.

Before the family exits from shelter, the Stabilization Case Manager will have a discussion with the family, Family Shelter Case Manager, Housing Specialists to determine what supports the family identifies as necessary to help them stay housed after they leave shelter, as well as the frequency of post-shelter communication.

Enter a family’s initial stabilization plan and progress into ETO/ASIST and update every 30 days until the stabilization services have ended.

Have at least a monthly communication with the landlord to confirm that the family is paying rent and to identify any issues that have arisen.

Document stabilization efforts and meetings utilizing the EA Case Notes in ETO/ASIST.

Track and record client’s demographics, income, immigration status, Hildebrand tiering system and other key measurable data points as requested on a weekly and monthly basis.

Collect written success stories of client experiences.

Help the family to access and utilize all available public and private resources to support housing retention.

Encourage each family to progress with the identified goals in their stabilization plan.

Cultivate successful landlord-tenant relationships, including monthly communication, and provide mediation if necessary.

Advocate for each family in any housing court appearances.

Intervene if the tenancy is endangered, for any reason.

Assist families in securing appropriate and adequate medical services and benefits with a primary health care provider.

Monitor the school attendance and educational progress for each school-age child in the family.

Connect families and guide them through first time homeowners' programs.

Engage stabilization clients in a peer-to-peer Hildebrand Alumni program.

Document progress in ETO/ASIST on a monthly basis utilizing the Stabilization touchpoint.

Consult with System Navigators if referrals to certain services are proving challenging.

The Stabilization Case manager must work with families in their stabilization caseload to ensure that 90% of families will maintain stable housing for at least 6 months after placement, 80% of families will maintain stable housing for at least twelve (12) months after placement, and 70% of families will maintain stable housing for at least twenty-four (24) months after placement.

If the Hildebrand is logistically unable to provide stabilization services based on the geographic location of the family’s rehousing, the Stabilization Case Manager must ensure the successful transfer of stabilization services to the appropriate HomeBASE administering agency.

Initiate intensive case management as needed internally and with HLC as needed
