Job summary

Battle Creek Health Care Center is seeking a Social Service Worker to lead the Social Services Department and coordinate care for residents. The role focuses on MDS assessments, admissions support, and overseeing Medicaid processes in a long‑term care setting.

We value empathy, integrity, and strong communication, with daytime hours and occasional evenings/weekends as needed. Bachelor's degree in Social Work and 2+ years of related experience are required, with state certifications as applicable.

Qualifications

• 2+ years of postgraduate experience in long-term care or related programs.
• Must meet state requirements for social work practice or hold NASW credential if state has no regulation.
• 2+ years of social work and MDS assessment experience preferred.

Requirements:

• Two years of postgraduate experience in long-term care or related programs

• Must meet equivalent state requirements for social work practice, or hold certification/credentialing from the National Association of Social Workers if the state has no legal regulation

• BS in Social Service required

• 2+ years of social work and MDS assessment experience, ideally in a long-term care setting

• Energetic, results-oriented team leader with superior interpersonal and communication skills

• Self-motivated, caring approach, with passion for working with elderly and behavioral residents
