Job summary

Live Well Homecare is seeking a Speech Language Pathologist (SLP) to provide in-home assessment and treatment for patients with hearing, swallowing, and language impairments. Responsibilities include evaluating speech and language abilities, developing and implementing rehabilitative services, and documenting progress.

You will educate caregivers and family members to support patient improvement and ensure high-quality, patient-centered care in home health settings.

Qualifications

  • Master's Degree in Speech-Language Pathology or Speech Therapy.
  • Current unrestricted state licensure as an SLP.
  • CPR/AED/BLS/First Aid certification.
  • At least one year of professional experience in a home health or similar setting.

Responsibilities

  • Provide patient-centered speech and language therapy services.
  • Perform evaluations, create treatment plans, and administer diagnostic and therapeutic techniques.
  • Maintain accurate clinical records and progress notes.
  • Educate caregivers and family members on supporting patient needs.
