SPEECH THERAPIST/SPEECH LANGUAGE PATHOLOGIST (ST PRN) PER DIEM

GET PAID TO WORK FROM HOME! This position is remote, but will require a valid driver’s license, proof of insurance, and a reliable personal automobile for local travel to provide care to clients in the DFW metropolitan areas.

Job Summary: Qualicare is looking for a highly skilled and compassionate Speech Therapist to join our team! As a Speech Therapist, you will play a vital role in delivering exceptional patient care, promoting physical function, and enhancing overall health and well-being. We offer not only geriatric care, but we also offer Hospice, Palliative, discharge to home, Alzheimer’s and Dementia care, and concierge medical care.

Responsibilities:

1. Evaluating levels of swallowing, speech and language difficulty.
2. Identifying treatment options.
3. Creating and carrying out individualized treatment plans addressing specific functional needs.
4. Teaching patients how to make sounds while improving their voices and maintaining fluency.
5. Helping individuals improve sentence structure and vocabulary used in written and oral language.
6. Working with adults and children in developing and strengthening muscles used to swallow.
7. Conduct comprehensive assessments of patients' physical, cognitive, and emotional abilities to identify areas of strength and weakness.
8. Develop and implement individualized treatment plans that incorporate speech therapy, physical therapy, and other modalities to improve patients' functional abilities and independence.
9. Utilize medical terminology to accurately document patient progress, treatment plans, and outcomes.
10. Collaborate with patients, families, and healthcare professionals to develop and implement treatment plans that address patients' unique needs and goals.
11. Provide patient education and support to help patients and families navigate the healthcare system and manage their condition.
12. Assist with case management by communicating with healthcare providers, social workers, and other stakeholders to ensure patients receive comprehensive care.
13. Demonstrate a commitment to critical care and patient safety by adhering to hospital or clinic protocols and standards.
14. Maintain a professional demeanor and contribute to a positive work environment by being respectful, empathetic, and supportive of colleagues and patients.
15. Counseling individuals and families on ways to cope with swallowing and communication disorders.
16. Provide direct patient care to a diverse range of patients.
17. Conduct thorough evaluations of patients to identify physical impairments and develop personalized treatment plans.
18. Implement and monitor patient treatment plans, adjusting as necessary.
19. Collaborate with healthcare professionals, including physicians, to ensure comprehensive patient care.
20. Stay current with the latest developments in speech therapy.
21. Document patient progress and maintain accurate medical records.
22. Educate patients on proper exercise techniques and provide guidance on maintaining a healthy lifestyle.
23. Collaborate with other healthcare professionals to develop and implement individualized care plans.
24. Provide emotional support and educate patients and their families on disease processes and treatment options.
25. Assist in the development and implementation of policies and procedures related to patient care.
26. Participate in quality improvement initiatives and contribute to the continuous improvement of patient care.

QUALIFICATIONS:

1. Minimum one year experience in a home healthcare setting.
2. One-year experience in geriatric care, preferred.
3. Experience working in a hospital or clinic setting.
4. Ability to provide patient care in a variety of settings, including home community care.
5. Excellent communication and interpersonal skills.
6. A passion for delivering high-quality patient care and a commitment to ongoing professional development.
7. Strong knowledge of medical terminology and ability to communicate effectively with patients and healthcare professionals.
8. Experience with case management and patient service.
9. Experience working with patients with a range of diagnoses, including those requiring critical care.
10. A commitment to ongoing professional development and a desire to stay current with best practices in occupational therapy.
