Overview

Works as part of the Home Health team to assess, plan, and treat patients with rehabilitative needs. Provides effective treatment plans in the patient's home under the Plan of Care. Models appropriate behavior aligned with MLH Mission, Vision, and Values.

Responsibilities

  • Performs initial patient assessments and re-assessments.
  • Develops and implements effective treatment plans according to the Plan of Care.
  • Identifies patient-specific learning needs and provides education accordingly.
  • Maintains documentation, tracks, and monitors cases reliably.
  • Contributes to treatment plan oversight as part of a collaborative team, operating within scope of practice.
  • Performs other duties as assigned.

Qualifications

Education

  • Master's Degree in Speech Pathology

Experience

  • No prior work experience required

  • Thorough knowledge of current theories and principles in speech pathology.
  • Excellent verbal and written communication skills.
  • Ability to interact effectively with peers, medical staff, patients, and families.

Licenses and Certifications

  • Speech Pathologist license in Tennessee and Mississippi, as applicable.

Supervision

There are no supervisory responsibilities for this position.

Physical Demands

  • Activities may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motions.
  • Must have good balance and coordination.
  • Physical exertion may involve lifting up to 50 lbs occasionally and 25 lbs frequently.
  • Requires close visual acuity for patient assessments and data analysis.
  • Frequent patient contact and exposure to various environmental risks.
  • Must have a valid driver’s license and insurance for travel to patient residences.
