Job summary

South Creek Post Acute in Washington is seeking a licensed Speech-Language Pathologist to join our patient‑centered care team as a Speech Therapist (ST). You will evaluate residents, develop individualized treatment plans, and provide skilled therapy per physician orders while collaborating with the care team.

Eligible candidates hold a current WA SLP license, a Master's in Speech-Language Pathology, and CCC-SLP is preferred.

Qualifications

• Current WA Speech-Language Pathologist license required.
• Master’s degree in Speech-Language Pathology from an accredited program.
• CCC‑SLP certification (preferred).

Responsibilities

• Evaluate residents for speech, language, cognitive, communication and swallowing disorders.
• Develop and implement individualized treatment plans to improve outcomes.
• Provide skilled therapy services per physician orders and care plans.
• Collaborate with physicians, nursing staff, families and the care team.
• Maintain accurate documentation and ensure regulatory compliance.
• Educate residents, families and caregivers on treatment strategies and progress.
