Job summary

Palmshomecare is seeking a dedicated Speech Language Pathologist to provide in-home therapy services in Florida. The role involves independent work with coordination of care with an interdisciplinary team.

The ideal candidate will have home health experience, Florida SLP licensure, and CPR certification, with reliable transportation for travel to patients’ homes.

Qualifications

• Master’s degree in Speech Language Pathology required.
• Active Florida SLP license required.
• Home health experience preferred.
• Knowledge of OASIS documentation is a plus.
• CPR certification required.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Perform comprehensive evaluations and OASIS assessments.
• Develop and implement individualized care plans.
• Treat patients with speech, language, cognitive, and dysphagia disorders.
• Accurately document visits in accordance with Medicare guidelines.
• Educate patients and caregivers on exercises and compensatory strategies.
• Communicate effectively with physicians and care team members.
