Job summary

Diamond Care Group is seeking a Speech Language Pathologist to provide in-home therapy services to patients in the Tampa area. The role requires independence and coordination with an interdisciplinary team, with a focus on comprehensive evaluations and personalized care plans.

The ideal candidate will have a Master’s degree in Speech Language Pathology, an active Florida SLP license, CPR certification, and a valid driver’s license.

Qualifications

• Master’s degree in Speech Language Pathology is required.
• Active Florida SLP license is required.
• Home health experience preferred.
• Knowledge of OASIS documentation is a plus.
• CPR certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Perform comprehensive evaluations and OASIS assessments.
• Develop and implement individualized care plans.
• Treat patients with speech, language, cognitive, and dysphagia disorders.
• Accurately document visits in accordance with Medicare guidelines.
• Educate patients and caregivers on exercises and compensatory strategies.
• Communicate effectively with physicians and care team members.
