We are a 143 bed Skilled Nursing Facility located in Port Jefferson Station, NY. We are seeking a Speech-Language Pathologist to provide services for disorders of speech, voice, swallowing, and language. Plans appropriate Speech-Language Pathology services and provides treatment in accordance with accepted professional standards and participates in the general functioning of the rehabilitation therapy department.

Responsibilities:

• Documents resident progress toward Speech-Language Pathology services.
• Complies with Federal and State regulations related to rehabilitation therapy.
• Demonstrates working knowledge of Medicare Part A PPS PDPM case management.
• Completes all documentation to comply with professional standards and regulatory requirements.

Requirements/Qualifications:

• Demonstrates working knowledge of skilled restorative therapy and maintenance programs.
• Screens residents for rehabilitation therapy needs.
• Completes evaluations upon physician order.
• Communicates and works together across departments.

Education:

Bachelor's Degree
