Job summary

Indiana Fitness Works, Inc. is seeking a Speech Language Pathologist (SLP) for a position in a long term care setting.

The SLP will be responsible for evaluation and treatment of communication disorders, swallowing disorders, and cognitive deficits, as prescribed by the physician. Responsibilities include conducting evaluations, identifying equipment needs, developing and revising plans of care, educating patients and families, prepare notes per agency policy, and performing other duties to

Qualifications

• Licensed Speech-Language Pathologist in Indiana.
• Experience in long-term care settings.
• Vaccination against Covid-19 or approved exemption is required.

Responsibilities

• Identify functional speech defects and establish a plan of care to improve patient’s function.
• Implementation and revision of plans of care as necessary.
• Provide education to patient, family/caregivers, and interdisciplinary team on recommendations for patient’s care.
• Prepare and submit notes per agency policy.
• Perform other duties to facilitate delivery of speech language pathology services.
