Job summary

Indiana Fitness Works, Inc. is seeking a Speech Language Pathologist (SLP) for a long-term care setting in Winchester, IN at Pine Knoll.

The SLP will evaluate and treat communication disorders, swallowing disorders, and cognitive deficits as prescribed by the physician. Responsibilities include conducting evaluations, identifying needs, planning and revising care, educating patients and families, preparing notes, and delivering comprehensive speech language pathology services.

Responsibilities

• Conduct evaluations, assess home environment (as appropriate).
• Identify functional speech defects and establish a plan of care to improve patient’s function.
• Implementation and revision of plans of care as necessary.
• Provide education/instruction to patient, family members/caregivers, and interdisciplinary team on recommendations for patient’s care.
• Prepare and submit notes per agency policy.
• Perform other duties as required to facilitate delivery of speech language pathology services.
