Job summary

Health Here is seeking a Speech-Language Pathologist to evaluate and treat patients in a home health setting. You will develop individualized plans and deliver therapy across all ages, from infants to geriatrics, adapting goals as assessments evolve.

Collaborating with the care team and families, you will document progress and contribute to program development, with opportunities to supervise clinical interns as part of a collaborative environment.

Qualifications

• ASHA CCC-SLP or active state license required.
• Ability to work effectively, communicate clearly, and demonstrate visual acuity.
• Preferred 1–3 years of experience in hospital, SNF, outpatient, or home health settings.

Responsibilities

• Evaluate, develop, and implement individualized treatment plans.

Speech-Language Pathologist

Role

Evaluate, develop, and implement individualized treatment plans for patients with speech, language, cognition, voice, and dysphagia deficits in a home health setting.

Responsibilities Include

• Performing diagnostic assessments and interpreting results to determine patient needs.
• Providing direct therapy tailored to patients across all age groups, from infants to geriatrics.
• Establishing and adjusting intervention goals and strategies based on ongoing evaluations.
• Collaborating with interdisciplinary team members, patients, and families to coordinate care.
• Documenting evaluations, treatment plans, and progress notes accurately.
• Participating in program development and supervising clinical interns.

Qualifications

• Certification from ASHA (CCC-SLP) or active state license.
• Ability to work effectively, communicate clearly, and demonstrate visual acuity.
