Overview

All About You Collaborative Health Care Services LLC is a nurse‑owned, transitional home‑care agency dedicated to helping patients transition from hospital or facility to home. We are looking for experienced and dedicated Home Health Speech Language Pathologists for per‑diem work.

Duties
  • Conduct evaluations and assessments of patients to determine speech and language disorders
  • Develop and implement individualized treatment plans for patients with speech and language disorders
  • Provide speech therapy services to patients with various conditions, such as neurological disorders, developmental delays, or communication impairments
  • Utilize a variety of techniques and modalities to improve speech, language, and swallowing abilities
  • Collaborate with other healthcare professionals to coordinate patient care and ensure optimal outcomes
  • Maintain accurate and up‑to‑date documentation of patient progress and treatment plans
  • Provide education and support to patients, families, and caregivers regarding communication strategies and techniques
Qualifications
  • Experience in conducting patient assessments and developing individualized treatment plans
  • Familiarity with medical terminology and documentation practices
  • Ability to work effectively in a multidisciplinary team environment

Requirements
