Job summary

Midwest Health, Inc. seeks a Speech Pathologist to assess, diagnose, treat and help prevent speech, language, cognitive-communication, voice, swallowing, fluency and related disorders in residents.

You will follow facility policies, document treatments, maintain confidentiality, and communicate treatment plans with the referring physician and rehabilitation team. Strong assessment and treatment planning skills are essential, and you will provide in-service education to staff as needed.

Qualifications

• Must hold a valid licensure as Speech-Language Pathologist in the state of practice.

Responsibilities

• Report witnessed or reported allegations of resident abuse/neglect immediately to the Administrator or Director of Nursing.
• Treat residents, family members, visitors, and team members with dignity and respect.
• Maintain the confidentiality of all resident and family information.
• Report to work on time and as scheduled.
• Identify patients with communication and swallow deficits.
• Evaluate patients within one business day of written referral from the Doctor.
• Plan and administer appropriate treatment and communicate with the referring physician and rehabilitation team.
• Maintain accurate daily records of treatments including billing and charting.
• Follow weekly documentation guidelines for all residents treated.
• Provide in-service education to facilities, supportive staff, students and community.
• Attain an efficiency standard for delivery of speech language pathology services.
